6. Registered Chaperone Application Form
If you wish to apply to become a registered Chaperone, please complete the below application form and return to Tom Thorp, Juvenile Employment and Education Welfare Officer at Cornwall Council, together with 2 x recent passport sized photographs of yourself. As part of this process, a DBS check (the new CRB) will be carried out, together with a short interview with Tom Thorp. Registered chaperones are required for performances, and I hope that you understand that the safety and well-being of the young people in Stagezone Youth Theatre is of paramount importance.
